SRH Awards Ballot

You may be prompted to take a survey from an advertiser when you complete your ballot. Don't worry; you are not required to fill out that survey. Just click "Continue" at the top of the page after the status bar is full and the words "Form Sent" appear.

Please complete this ballot by 3:00pm on Thursday, May 29th, 2025.

1. Under no circumstances may you vote for yourself for any award. Voting for yourself or making a mockery of the ballot may cause your entire ballot to be voided.
2. Please be certain to look over the entire list of players that are eligible for each award. Don't risk forgetting someone who deserves an award just because you didn't read carefully.
3. Be sure to completely fill out each portion of the ballot and submit it by Thursday, May 29th at 3:00PM CT. There are 12 awards on which your vote is needed. On each award, you will be asked for your top three choices. Please give me your top three choices for each award by entering your 1st, 2nd and 3rd place votes separately for each award. Be sure not to vote for the same person twice for any award.
4. You must give me your name and e-mail at the beginning of your ballot. Don't worry; no one will know how you voted except for me. You're certainly welcome to tell people how you voted, but I will not tell anyone for you.
5. Be certain to look at statistics for 2024-2025 when filling out your ballot by going to www.sundayrollerhockey.com/page7, but remember that statistics are not the only measuring stick you should use to come to a conclusion.
